ABSTRACT

Background: The diabetic ulcer incidence in type-2 diabetes mellitus (DM) patients may increase due to Dislipidemia . This study was describing the differences in diabetic ulcer incidence between  dislipidemia and normolipidemia  type-2 DM patients that were hospitalized at Kariadi hospital during January 1, 2001 until December 31, 2003.

Methods: This study was a cross-sectional study, with Kariadi Hospital medical records (MR) as the population. The samples were the MR of diabetic ulcer in type-2 DM patients, who are hospitalized, and complying with the inclusion and exclusion criterias. The data were analyzed using  Mann-Whitney test for differences.

Results: There were 66 dislipidemia (84.62%) in 78 patients with diabetic ulcer sample which consists of 23 with  hypercholesterolemia, 53 with hypertrigliceridemia, and 10 samples with both. Also 12 (15.38%) ulcers occured with normolipidemia. The cholesterol blood level of dislipidemia type-2 DM with diabetic ulcer was 181.35±26.5 mg/dl, and the trigliceride level  was 179.58±60.9 mg/dl. The cholesterol blood level of normolipidemia type-2 DM with diabetic ulcer was 147.25±27.4 mg/dl, and the trigliceride level was107.92±28.5 mg/dl. The Mann-Whitney test for cholesterol was p=0,045, and the  triglyceride was p=0,002.

Conclusions: The incidence of diabetic ulcer in type-2 DM patients with dislipidemia was higher than normolipidemia. The cholesterol and triglyceride blood  level were higher in dislipidemia.

ABSTRAK

Latar belakang: Insidensi ulkus diabetes dapat meningkat apabila penderita DM tipe-2 menderita dislipidemia. Penelitian ini menjelaskan perbedaan angka kejadian ulkus diabetes pada pasien DM tipe-2 dengan dislipidemia atau tidak, yang dirawat di RS. Dr. Kariadi periode 1 Januari 2001 sampai 31 Desember 2003.

Metoda: Penelitian observasional dengan pendekatan cross sectional ini mengambil populasi dari rekam medik (RM) RS Dr. Kariadi. Sampel penelitian adalah RM penderita DM tipe-2 yang dirawat di bangsal penyakit dalam RS Dr. Kariadi yang memenuhi kriteria inklusi dan ekslusi. Data diedit, ditabulasi, dideskripsikan, diolah dengan uji  Mann-Whitney.

Hasil: Ditemukan 66 penderita (84,62%) dislipidemia dari 78 penderita ulkus diabetes yang terdiri atas 23 penderita hiperkolesterolemia, 53 penderita hipertrigliseridemia, dan 10 penderita hiperkolesterolemia disertai hipertrigliseridemia. Tidak terdapat dislipidemia, duabelas penderita (15,38%) sisanya.  Kadar kolesterol penderita ulkus diabetes dengan dislipidemia 181,35±26,5 mg/dl dengan kadar trigliserida 179,58±60,9 mg/dl. Kadar kolesterol pada penderita tanpa dislipidemia 147,25±27,4 mg/dl dengan kadar trigliserida 107,92±28,5 mg/dl. Uji Mann-Whitney untuk kadar kolesterol pada penderita ulkus diabetes dengan dislipidemia p=0,045, sedangkan untuk kadar trigliserida p=0,002.

Kesimpulan: Angka kejadian ulkus diabetes pada pasien DM tipe-2 dengan dislipidemia lebih tinggi dibanding normolipidemia. Kadar kolesterol dan trigliserida lebih tinggi secara bermakna pada penderita ulkus diabetes dengan dislipidemia.
